| When completed, with a passing score, come to the Health Department (1005 W Worley) between 8:00 AM - 4:30 PM, Monday through Friday, except for holidays. You will need to pay $15 at this time (with cash, check, or card), and your card will be printed. If you serve or sell alcohol, you will also need to complete the SMART training found at smart.missouri.edu, and have this available to view (screen on phone or printed copy), and the training will be added to your card at no additional cost. |
